Diablo 4 Season 1 added Malignant enemies to Sanctuary and instead of directly dropping gear or gems, these enemies drop hearts that can be socketed into your jewelry such as Rings and Amulets. Every class in the game has some exclusive hearts while a few options are available to all classes.

While getting your hands on Vicious, Brutal, and Devious Hearts is an effortless task, unlocking Wrathful Hearts for your characters requires some grinding. As a result, Wrathful Hearts are significantly more powerful and one such sought-after heart is the Caged Heart of the Barber.

Diablo 4 Season 1 has four types of Malignant Hearts that can be altered to get Caged Hearts, which are items that can be socketed into jewelry to obtain powerful buffs. The Caged Heart of The Barber is a Wrathful Malignant Heart that grants powerful Defensive buffs.

When using the Caged Heart of The Barber, Critical Strikes and all subsequent damage within 2.0 and 4.0 seconds are absorbed by your target. The absorbed damage then erupts onto surrounding enemies, and store damage is increased by 10% per second.

The Caged Heart of The Barber can be dropped randomly after defeating any Wrathful Malignant Enemies in Diablo 4 Season 1. These will appear only when playing at World Tier 3 or higher.

In the case you’re looking to farm the items, you’ll be able to look for the Caged Heart of The Barber by using an Invoker on a Malignant Outgrowth at the end of the Malignant Tunnel.
