You can find the Poison Mist in Elden Ring in the forest southeast of the Castle Morne Rampart Site of Grace. There’s a teleporting Teardrop Scarab that you must kill to obtain the item and ranged attacks are best to take it down with ease.

In melee combat, the Scarab might teleport if you don’t kill it in time. Don’t worry, though; it only teleports to nearby regions and can be spotted easily because of its glowing texture.
